RMN or Social Worker (Band 6)
Job Overview
We are seeking an RMN or Social Worker to work as a keyworker, managing a caseload of patients with varying mental health conditions and diagnoses. The successful candidate will provide mental health assessments, support community mental health nursing, and manage medication for patients. Knowledge of duty diary management and experience in CMHT and the new transformation model is desirable. Familiarity with RIO is a plus.
Responsibilities
- Act as a keyworker managing a caseload of patients with varying mental health conditions.
- Conduct mental health assessments and create individualized care plans for patients.
- Provide support in community mental health nursing, including medication management.
- Manage duty diary and ensure efficient handling of patient cases.
- Collaborate with the multi-disciplinary team and contribute to the CMHT transformation model.
- Monitor patient progress and adapt care plans as needed.
Requirements
- RMN or Social Worker qualification with valid professional registration.
- Experience in managing mental health conditions and conducting mental health assessments.
- Knowledge of community mental health nursing and medication management.
- Familiarity with RIO is desirable.
- Experience with CMHT and understanding of the new transformation model is a plus.
- Excellent communication skills and ability to work in a multi-disciplinary team.
