Job Overview
We are seeking a Specialist Memory Assessment Nurse (RMN) to join the multi-disciplinary Memory Assessment Service (MAS) as an independent specialist practitioner. The role involves conducting assessments, planning, consultations, and diagnostic formulations for individuals with memory concerns. The successful candidate will have specialist knowledge in dementia treatment strategies and interventions, ensuring effective risk identification and management, and facilitating referrals to appropriate services. Additionally, the role includes mentoring staff and trainees, acting as a liaison between GPs, LCFT, and non-statutory services, and supporting the team manager in leadership and service development while upholding Trust values and professional standards.
Responsibilities
- onduct specialist memory assessments and contribute to the diagnostic process.
- Develop and implement individualized care plans in collaboration with the MAS team.
- Provide expert advice and interventions for dementia care and treatment.
- Identify risk factors, manage safeguarding concerns, and take appropriate action.
- Facilitate referrals to relevant services for further support.
- Act as a liaison between GPs, LCFT services, and non-statutory agencies to ensure coordinated patient care.
- Offer mentoring, supervision, and training to staff and trainees.
- Support the team manager in leadership, planning, and monitoring of patient care.
- Promote Trust values and adhere to NMC and HPC codes of conduct.
Requirements
- Registered Mental Health Nurse (RMN) with valid NMC registration.
- Experience in dementia care and memory assessment services.
- Strong clinical assessment, risk management, and care planning skills.
- Ability to work independently and collaboratively within a multi-disciplinary team.
- Experience in referring to appropriate services and working within healthcare systems.
- Excellent communication and leadership skills.
- Knowledge of current dementia treatment strategies and interventions.
- Commitment to mentoring and supervising junior staff and trainees.
