RMN, Occupational Therapist, or Social Worker – Community Mental Health (Band 5 or 6)
Job Overview
We are seeking an experienced Registered Mental Health Nurse (RMN), Occupational Therapist (OT), or Social Worker (SW) to provide specialist support for individuals with complex mental health needs in a community setting. The role requires expertise in assessing, planning, implementing, and reviewing care pathways, conducting risk assessments, and working as part of a multidisciplinary team. The successful candidate will manage a caseload of service users and provide care both at the base and in the community—a car driver is essential.
Responsibilities
- Conduct comprehensive assessments and develop personalized care plans for individuals with mental health issues.
- Manage a caseload of service users with varying mental health conditions.
- Assess risk, implement risk management strategies, and adapt care plans accordingly.
- Work collaboratively within a multidisciplinary team to provide holistic care.
- Address the impact of health and social factors on mental health and well-being.
- Provide support and interventions both at the base and within the community.
- Maintain accurate documentation and ensure care plans are regularly reviewed and updated.
Requirements
- Registered Mental Health Nurse (RMN), Occupational Therapist (OT), or Social Worker (SW) qualification.
- Proven experience working with individuals experiencing complex mental health problems.
- Strong knowledge of risk assessment and management.
- Ability to work both independently and collaboratively within a multidisciplinary team.
- Community experience preferred, with the ability to adapt to various settings.
- Full UK driving license and access to a vehicle for community visits.
