Social worker / Mental Health nurse (Band 6)
Job Overview
We are seeking a dedicated Social Worker / Mental Health Nurse to support young people with a variety of mental health and wellbeing needs. The role involves conducting independent assessments, creating care and discharge plans, and working with risk management in a CAMHS environment. The ideal candidate will have experience in triaging, safeguarding, and supporting multidisciplinary teams to ensure comprehensive care and treatment.
Responsibilities
- Conduct independent assessments and develop care and discharge plans.
- Manage and adapt risk plans based on changing circumstances.
- Provide triage and duty support, prioritizing caseload and tasks.
- Support a multidisciplinary team in the delivery of care.
- Work with young people with mental health issues, including CAMHS and child protection work.
- Signpost to appropriate services and make safeguarding referrals when necessary.
Requirements
- Experience in working with young people with diverse mental health and wellbeing needs.
- Proven ability to work independently and in collaboration with teams.
- Experience in CAMHS, child protection, and managing risk.
- Must be face-to-face workers only.
