Community Dietitian - (Band 6)
Job Overview
We are seeking a dedicated Community Dietician to provide dietetic assessments, advice, and care planning to community patients, particularly vulnerable elderly individuals with specific nutritional needs. This role involves working closely with a multidisciplinary team (MDT) to ensure safe and effective care for patients, including those with HEF (High Energy Food) requirements.
Responsibilities
- Provide dietetic assessments and advice for community patients, particularly the elderly.
- Collaborate with other healthcare professionals to deliver multidisciplinary care.
- Ensure that patients with nutritional needs are receiving appropriate, safe, and effective care.
- Work with HEF patients to enhance nutritional intake and improve health outcomes.
Requirements
- Experience in community dietetics and working with vulnerable elderly patients.
- Ability to work as part of a multidisciplinary team.
- Strong knowledge of nutritional needs and care planning.
- Remote work is not an option; the role requires face-to-face patient interaction.
