Job Overview
We are seeking a dedicated Social Worker / Mental Health Nurse to provide face-to-face support for young people with diverse mental health and wellbeing needs. The role involves independent assessments, risk management, discharge planning, and collaborative team support within established systems and thresholds.
Responsibilities
- Conduct independent assessments and provide tailored support for young people.
- Work within thresholds and systems, ensuring effective care and hospital discharge planning.
- Manage risk assessments, adapting plans to evolving needs.
- Provide triage/duty support, prioritizing caseloads efficiently.
- Collaborate with multidisciplinary teams and support service-wide initiatives.
- Signpost young people to appropriate services and make safeguarding referrals when necessary.
- Work with CAMHS and child protection services, ensuring high-quality care and intervention.
Requirements
- Qualified Social Worker (SWE registered) or Mental Health Nurse (NMC registered).
- Experience in CAMHS, child protection, or young people’s mental health services.
- Strong skills in risk assessment, care planning, and crisis intervention.
- Ability to work independently and within a multidisciplinary team.
- Face-to-face engagement is essential for this role.
