Job Overview
We are seeking an experienced Occupational Therapist for the SMASH (Specialist Multidisciplinary Assessment & Support at Home) service, with strong community experience and the ability to manage an independent caseload. The successful candidate will work with patients in their homes, providing personalized occupational therapy to support their daily living activities. Car driver is essential for this role, as it requires travel to multiple locations within the community
Responsibilities
- Manage an independent caseload, providing assessment and treatment for patients in the community.
- Develop and implement personalized care plans to improve patients' independence and quality of life.
- Work closely with other healthcare professionals to deliver holistic patient care.
- Provide ongoing support and review to ensure patient progress.
- Maintain accurate patient records and documentation.
Requirements
- Degree in Occupational Therapy (HCPC registration required).
- Experience in community-based occupational therapy.
- Ability to manage an independent caseload effectively.
- Car driver (essential for home visits).
- Strong communication, organizational, and problem-solving skills.
