Job Overview
An exciting opportunity has arisen for a Band 6 Occupational Therapist (OT) or Physiotherapist (PT) to join a dynamic community team. This role involves delivering high-quality rehabilitation and therapy services to patients in their own homes and community settings, promoting independence and improving quality of life.
Responsibilities
- Community-Based Care: Provide specialist OT/PT assessments and interventions for a diverse patient caseload, supporting mobility, function, and daily living activities.
- Holistic & Patient-Centered Approach: Work collaboratively with patients, families, and multidisciplinary teams to develop individualized care plans.
- Autonomous & Team Working: Manage a caseload independently while contributing to a supportive and effective therapy team.
Requirements
- NHS experience is mandatory.
- HCPC registration as an Occupational Therapist or Physiotherapist.
- Full UK driving license and access to a vehicle for community visits.
- Full-time preferred, but part-time hours will be considered.
